After a stroke the brain rewires most aggressively during the first ninety days. Therapy started in this window produces noticeably better long-term walking and hand function.
Sessions should be frequent rather than long. Three to five short sessions a week beat one long weekly visit, which is exactly why home-visit therapy works so well for stroke patients.
Families play a huge role. We train one caregiver in each household on transfers, positioning and a short daily exercise set to repeat between visits.
Progress is rarely linear. Plateaus are normal, and the goal shifts over time from movement to independence in daily tasks like dressing, eating and using the bathroom.
